WRAPAROUND COORDINATOR:  Contractual Wraparound Coordinator for children that are multiple services across different agencies, whom without these services would be at risk of hospitalization. The Coordinator’s responsibilities include, but are not limited to, receiving wraparound service referrals from community partners, gathering/completing demographic and financial documentation, and screening for eligibility for enrollment in the Serious Emotional Disturbance Waiver Program. The Coordinator will work closely with the community and family team to identify family strengths and coordinate community resources and treatment services.  Minimum of a bachelor degree with 3 years of experience working with children, or a master’s degree with at least 1-year experience.  Pay range is $16 – $18 an hour depending on degree, and includes paid training and flexible hours.
